Advance Directives Documents Available
While no one wants to think about losing the ability to make their own medical decisions, advance directives – documents that communicate what kind of care a person wants should they become unable to make medical decisions – can offer some piece of mind for everyone involved in a situation where those directives are needed.
You can help your patients secure advance directives by providing them with copies of the Illinois State Medical Society’s recently revised A Personal Decision document. The booklet contains important information on living wills, health care powers of attorney, and do not resuscitate orders. The official "Illinois short form power of attorney for health care" document is also included.
